Ann Kozlowski

Ann Dora Kozlowski, 71, of Peoria, Arizona passed away October 2, 2016. Ann was born in Chicago, Illinois. A homemaker, she resided in Arizona for the past 17 years and was a member of St. Elizabeth Seton Catholic Church. She is survived by her husband of 51 years, William "Bill" Kozlowski, sons William A. Kozlowski (Kim Pierce) and Mark Kozlowski (Angie), grandchildren Jaclyn and Alexius Kozlowski. Her daughter Helen Marie Kozlowski precedes her. Visitation will be 4-7 p.m. with prayer vigil at 6:30 p.m. on Wednesday, Oct. 5, 2016 at Menke Funeral & Cremation Center. Funeral Mass will be held at 11:00 a.m., Thursday, Oct. 6, 2016 at St. Elizabeth Seton Catholic Church, 9728 W. Palmeras Dr., Sun City, Arizona.
